To keep you healthy and active, Southeast Community Care offers the following annual health screenings at no cost:
- Bone mass measurement
- Colorectal screening exam
- Mammograms
- Pap smears and pelvic exams
- Prostate cancer screening exams
Making Good Care... Better
When you join Southeast Community Care, you are not trading in your valued Medicare and State Medicaid coverage. You actually get more coverage through our coordinated care plan than with Original Medicare and still keep the same level of health care assistance that you receive from the state!
Your Health Care Team
With Southeast Community Care, your health care will be coordinated by you and your Primary Care Physician (PCP). Think of your PCP as the "quarterback" of care, who works directly with your team of specialists to determine the best strategy to support your health care needs. In many cases, you can keep your current doctor. Or you have the option to choose a doctor from our list of Dual Plus Plan doctors.

Southeast Community Care is provided by the Arcadian Health Plan of North Carolina, Inc., a Coordinated Care Plan with a Medicare Advantage Contract. The plan is available to anyone who has both Medical Assistance from the state and Medicare. You much continue to pay your Medicare Part B premium if not otherwise paid for under the Medicaid or by another third party. You must use contracted providers and pharmacies for car. Limitations, monthly premiums, copayment sand coinsurance and deductibles may vary based on income. Please contact the plan for further details.
